In order for us to dispatch all product returns, we ask you before
we can do it, that you correctly fill in this form, immediately after
being received in Ortoibérica, and the return accepted, it will be sent
back to you with an RMA number, which will allow the reception of the
merchandise in our warehouse. For damaged or subjected to warranty
products its absolutely necessary that you correctly fill in all the areas,
so we can dispatch them with the necessary speed.
